My Q&A doc got copy-pasted by the CEO, zero context

I put together a simple internal FAQ for our product launch last Monday, and the CEO pasted it verbatim into the all-hands email without checking the 'not approved yet' note I left at the top. It actually worked out fine because the tone was casual enough, but now I'm terrified every draft I write will get shipped early. Anyone else had leadership grab a working doc before it was final?
