DAE stumble over words when asking something important?

I tried to ask my doctor a question about a health worry last visit. My mind went blank and I just nodded instead, missing my chance. How do you keep your cool and ask what you need to?

Honestly, that happens to me more often than I'd like to admit. Last month at the dentist, I meant to ask about a tooth pain but ended up just agreeing to a cleaning. My mind goes blank under pressure every time. Writing a quick note on my phone before the appointment has saved me a few times. What tricks do you use to remember your questions?
