Is it only about quick fixes, or do bigger things like cost and convenience get in the way too? Those are harder to solve.

Totally... those bigger things are the whole problem most of the time. Quick fixes get talked about because they're easy, but cost and convenience are the real walls people hit. It's like companies focus on the small stuff because the big issues are too hard to change. They don't want to rebuild a whole system or cut into profits. So we keep getting bandaids when the actual wound is way deeper.
