I paid off a $500 medical bill from 2019 and my score dropped 15 points.

It was a small, old collection I'd forgotten about, and paying it suddenly made it a 'new' activity. Anyone else get punished for trying to do the right thing?

Credit scoring is a broken system. It makes no sense that fixing a past mistake counts against you. The whole thing feels designed to keep people trapped. Paying old debts should help, not hurt. They reward you for ignoring problems and punish you for trying to clean things up. It's backwards and frustrating for everyone.

Yeah, that happened to me too. Called the collector and asked for a "pay for delete" before sending any money. Got it in writing, then paid. Score bounced back after a few weeks. Did you try asking them to remove it?
