Cash handling flip after a $40 drawer shortage

I used to count my till twice at open and once at close, thought that was bulletproof, but last month I came up $40 short and my manager showed me the bank deposit slip was off because I never checked the coin rolls. Now I verify the coin amounts against the safe log every shift, has anyone else had a drawer mystery that was just loose change?
