DMV renewal took me 11 months because I missed one checkbox

Sent in my license renewal by mail. Got it back 4 months later saying incomplete. I missed the organ donor checkbox. Had to start the whole process over. Took 11 months total. Has anyone else had a simple mistake snowball like that?

Huh, that's brutal. Did they at least give you a clear explanation of what was missing or just a vague "incomplete" notice?
