Under extruded upper layers?

I just completed my first major print- 7 inches tall and about 300 grams. I used Cura. It's white 1.75mm PLA at .2mm layer height and took about 20 hours.

I don't know if you can tell from the photo, but the bottom layers are completely fused, and about 3 inches up it starts to get less and less fused as it gets higher.

The measured height is almost exactly what Cura said it would be- so I think the Z-calibration isn't the problem nor is z-axis movement.

Any thoughts on what would cause issues as the model got taller?

You can't really see that from the pic, can you post some close-ups of bottom and top.

This page is basically what I was seeing- except it would come and go. But I cleaned out the hotend and it instantly got a lot better.
